Description
Enerpeel TCA Strong 40% is the most powerful skin remodelling peel in the Enerpeel range, formulated with 40% trichloroacetic acid (TCA) in the patented Enerpeel carrier system. It is designed for profound, deep dermal remodelling to improve colour, freshness, texture and skin tone and to significantly reduce scarring, including ice-pick acne scars, as well as deep lines and wrinkles. Enerpeel TCA Strong 40% also helps remove superficial discolouration such as age spots and treats severe photoaging, chrono-aging, melasma, lentigines and actinic keratoses. The Enerpeel carrier restrains the surface activity of TCA and then drives it deeper into the skin, reducing surface trauma and making the procedure more controlled and predictable than traditional TCA peels of comparable strength. Formulation

Usage
- For professional in-clinic use only by appropriately trained medical or aesthetic practitioners familiar with medium-to-deep TCA peels.
- Conduct a full medical and dermatological assessment including history of scarring, pigmentation disorders, medications (e.g. isotretinoin) and Fitzpatrick skin type. Ensure extended skin preparation with appropriate topical agents as recommended for Enerpeel TCA Strong 40%, particularly in darker or pigment-prone skin.
- Cleanse the treatment area thoroughly to remove make-up, sebum and debris, then degrease using the recommended pre-peel solution or wipes (e.g. Enerpeel PS or equivalent).
- Protect sensitive sites (eyes, nostrils, lips, mucous membranes and any non-target areas) with appropriate barrier products such as petrolatum according to clinic protocol.
- Open a 2 ml vial of Enerpeel TCA Strong 40% and pour into a sterile dish or use directly with a suitable applicator brush. For full-face applications, typically 0.5–1 ml is used depending on area size and protocol; remaining solution is discarded after single use.
- Apply a uniform layer of Enerpeel TCA Strong 40% to the treatment area using a brush or applicator, working systematically and avoiding overlap pools of solution. Do not allow the product to run into protected areas.
- Observe the skin closely for the development of frosting (white coagulation) and patient-reported sensation. Typical contact times are around 1–4 minutes. A second or third layer may be applied to selected areas if indicated, ensuring that previously frosted areas are not re-treated to avoid excessive depth.
- Once the desired level of frosting and clinical end-point is achieved or if significant discomfort occurs, neutralise or rinse according to protocol: generally by washing with copious cool water and/or by using an appropriate neutralising agent and gently patting dry.
- After neutralisation/rinsing, apply post-peel products recommended by the manufacturer, which may include hydrocortisone creams to reduce erythema and topical antibiotic preparations until crusting resolves, typically applied several times daily until re-epithelialisation is complete.
- Inform the patient that erythema, swelling and crusting will develop over the following days, with skin typically taking about 10–14 days to return to a more normal appearance. Provide written post-peel instructions emphasising: no picking or scratching, use of bland cleansers and moisturisers, avoidance of heat and strenuous exercise initially, and strict broad-spectrum photoprotection with high SPF.
- For focal TCA CROSS treatment of deep ice-pick scars, load a small droplet of Enerpeel TCA Strong 40% onto a fine wooden applicator (e.g. toothpick) and carefully apply only to the base of the scar, avoiding surrounding skin; frosting should occur in the scar base only. Follow specific CROSS protocols and aftercare guidance.
- Do not repeat Enerpeel TCA Strong 40% full-face treatments more frequently than recommended (commonly 3–6 months or longer between deep peels), and adjust timing based on clinical response, healing and patient-specific risk factors.
Contraindications
- Known hypersensitivity or allergy to trichloroacetic acid or any component of the Enerpeel TCA Strong formulation.
- Active skin infections (bacterial, viral or fungal) in the treatment area, including active herpes simplex lesions.
- Open wounds, erosions, severe dermatitis or uncontrolled inflammatory dermatoses in the treatment area.
- Recent (within several months) ablative laser resurfacing, deep chemical peeling or surgical procedures in the proposed treatment area where healing is incomplete.
- History of keloid or hypertrophic scarring or significant wound-healing disorders, unless carefully evaluated and treated under strict medical supervision.
- Current or very recent systemic isotretinoin therapy where deep TCA peels are contraindicated according to local guidelines.
- Recent radiotherapy to the treatment area.
- Tendency to severe post-inflammatory hyperpigmentation, especially in Fitzpatrick IV–VI, unless the clinician judges benefits to outweigh risks and undertakes extended preparation and close monitoring.
- Pregnancy or breastfeeding, as a precaution, unless the supervising physician explicitly considers treatment acceptable under local regulations.
- Age below the threshold recommended by local regulation or manufacturer, and patients unable or unwilling to comply with strict pre- and post-peel care, particularly photoprotection and avoidance of picking.
Adverse Effects
- Expected acute effects include intense stinging or burning during application, pronounced erythema, skin whitening (frosting) and tightness.
- In the days following treatment, swelling, marked erythema, darkening of treated skin, crusting and exfoliation are typical and can be significant due to the depth of peel.
- Risk of prolonged erythema lasting several weeks, particularly in sensitive individuals.
- Post-inflammatory hyperpigmentation or hypopigmentation, especially in darker skin types or where photoprotection is inadequate.
- Potential for infection (bacterial, viral or fungal) of the treated skin if the barrier is not protected or aftercare is poor.
- Risk of scarring (atrophic, hypertrophic or keloid) if peel depth is excessive, if protocols are not followed, or in predisposed individuals.
- Persistent textural changes or demarcation lines between treated and untreated skin in cases of aggressive or uneven application.
- Rare allergic or idiosyncratic reactions to formulation components or to concomitant pre- or post-peel products.
Storage Conditions
- Store in a cool, dry place away from direct sunlight and sources of heat.
- Keep vials in their original packaging and tightly closed until use to minimise evaporation and maintain concentration.
- Observe any specific storage temperature recommendations provided in the Enerpeel documentation (typically controlled room temperature unless otherwise stated).

Healing following one peel generally involves 10\u201314 days of visible erythema, swelling and crusting before re-epithelialisation and progressive improvement over subsequent weeks and months.
Onset
Initial improvement in skin brightness and surface texture is usually visible after re-epithelialisation (approximately 2 weeks post-treatment), with more marked improvements in pigmentation, scarring and deeper lines appearing over the following 4\u201312 weeks as dermal remodelling progresses. Results from TCA CROSS treatments may be seen progressively over several months as scar tissue remodels.
