Description

Phenoxymethylpenicillin 250mg Tablet x28 is a prescription-only generic antibiotic pack containing 28 oral tablets, each with 250mg phenoxymethylpenicillin (Penicillin V) as the potassium salt. Phenoxymethylpenicillin is a narrow-spectrum beta-lactam penicillin used to treat and prevent mild to moderately severe infections caused by penicillin-sensitive organisms, particularly infections of the ear, throat, chest (respiratory tract), skin and soft tissue, and certain dental and prophylactic indications. This generic product is supplied for use in primary care and other clinical settings via Williams Medical Supplies and other UK medical wholesalers.

  • Dental and oro-facial infections caused by susceptible organisms, as per local antimicrobial guidelines.
  • Prophylaxis of recurrent rheumatic fever and/or chorea, as recommended in national or local protocols.
  • Prophylaxis against infection in certain at-risk patients (e.g. some patients with asplenia or sickle cell disease) when phenoxymethylpenicillin is specified in local protocols.
  • First-line treatment of acute sore throat of likely streptococcal origin in some UK community sore throat test-and-treat or PGD-based services, where phenoxymethylpenicillin 250mg tablets are specified by protocol.

Composition

  • Each tablet contains 250 mg phenoxymethylpenicillin (as phenoxymethylpenicillin potassium).

Usage

  • Use only as prescribed by a qualified healthcare professional and according to the instructions in the relevant Patient Information Leaflet.
  • Typical oral adult and adolescent dosing for treatment of infections is often 250mg to 500mg every 6 hours (four times daily), but exact dose, frequency and duration must follow the prescriber’s instructions and current clinical guidelines.
  • For sore throat/tonsillitis treatment protocols, adults and older children may receive 250mg to 500mg taken several times a day for around 10 days, where phenoxymethylpenicillin is recommended by local guidelines.
  • Tablets should generally be swallowed whole with water.
  • Many SmPCs advise taking phenoxymethylpenicillin on an empty stomach (for example, at least 30 minutes before food or 2 hours after food), as food may slightly reduce absorption; follow the specific advice in the PIL supplied with the pack.
  • Complete the full prescribed course, even if symptoms improve earlier, to reduce the risk of recurrence and antibiotic resistance.
  • If a dose is missed, take it as soon as remembered unless it is almost time for the next dose; do not double doses to make up for a missed dose.
  • Seek medical advice promptly if symptoms worsen, fail to improve as expected, or if significant side effects occur.
  • Do not use for viral infections such as the common cold or uncomplicated viral sore throat where antibiotics are not indicated.

Contraindications

  • Known hypersensitivity to phenoxymethylpenicillin, other penicillins, or any beta-lactam antibiotics (e.g. cephalosporins) due to risk of severe allergic reactions including anaphylaxis.
  • History of severe immediate hypersensitivity reaction (e.g. anaphylaxis) to any penicillin or cephalosporin.
  • Use in serious or life-threatening infections requiring parenteral therapy, or where oral absorption may be unreliable (e.g. severe systemic illness), unless directed by a specialist with appropriate monitoring.
  • Use in patients with known hypersensitivity or intolerance to any of the tablet excipients (such as lactose or particular dyes/binders) as listed in the relevant manufacturer’s PIL.
  • Caution (not absolute contraindication) is required in patients with significant renal impairment; dose adjustment or alternative therapy may be needed under medical supervision.
  • Caution in patients with a history of antibiotic-associated colitis.
  • Not to be used for infections caused by organisms known or strongly suspected to be resistant to phenoxymethylpenicillin.

Adverse Effects

  • Gastrointestinal disturbances such as nausea, vomiting, abdominal discomfort and diarrhoea.
  • Hypersensitivity reactions including rash, urticaria (hives), pruritus (itching) and, rarely, severe skin reactions.
  • Anaphylactic reactions, including angioedema and bronchospasm, which may be life-threatening and require immediate medical treatment.
  • Antibiotic-associated colitis, including Clostridioides difficile-associated diarrhoea, particularly with prolonged use or in susceptible individuals.
  • Transient changes in blood counts (e.g. eosinophilia, leucopenia, thrombocytopenia) reported rarely with penicillins.
  • Very rare hepatic effects such as elevations in liver enzymes or cholestatic jaundice.
  • Superinfection with non-susceptible organisms, including fungal overgrowth such as oral or vaginal candidiasis, with prolonged or repeated courses.
  • Headache or dizziness in some patients.
  • Any severe, persistent or worrying side effects should prompt medical review and possible discontinuation of treatment.

Storage Conditions

  • Store in the original packaging to protect from moisture and, where applicable, from light.
  • Keep out of the sight and reach of children.
  • Store at or below room temperature as specified in the PIL (commonly below 25°C, but follow the storage conditions stated for the specific product supplied).
  • Do not use after the expiry date printed on the carton and blister pack.
  • Do not use tablets that appear damaged, discoloured or otherwise deteriorated.

Duration

Duration of therapy depends on the type and severity of infection and national/local guidelines; common courses for acute infections such as streptococcal sore throat are around 10 days, while prophylactic use (e.g. in rheumatic fever) may be long-term or continuous, strictly as directed by a specialist.

Onset

Bacterial load reduction begins soon after adequate plasma concentrations are achieved; clinical improvement is often observed within 24\u201348 hours for susceptible infections, although full resolution requires completion of the prescribed course.
